Devoid of Earth and now finding his ties with SYSTEM irksome, the Medical professional and Sarah Jane headed for the stars.
While without the Earth, the Doctor was not complimentary of his own people and was frequently pushed into objectives by the Time Lords, most significantly sending him back in time to challenge the creator of the Daleks, Davros, in an effort to change them or clean them out completely.
Sarah Jane followed for a while, but in the end chosen to stay in the world while the Physician lost himself amoung the stars with a fellow Time Lady, Romana and a robotic pet dog, K-9.
It was during an effort to finally fix the TARDIS chameleon circuit that the Physician crossed swords with renegade Time Lord, The Master again and in stopping him from taking control of deep space fell from atop a radio telescope, much to the horror of his buddies Nyssa and Adric.
However an injury like that is never adequate to stop a Time Lord …

Tom Baker portrayed the Medical professional longer than anyone else on screen therefore is, possibly appropriately so, typically thought of as the quintessential Medical professional. Odd, slender, with his hallmark long headscarf, ever present bag of jelly children and a thriving voice, he was nearly as unusual as the aliens he battled.
He was the first Physician I remember, but possibly not quite ‘my’ Doctor as I often discovered his episodes quite frightening!
